- Q. Do men wishing penile
enlargement usually consult a urologist?
A. Yes.
- Q. Can a man's perception of
his penile size, even if it is normal, affect his
interactions with sexual partners and professional associates?
A. Yes.
- Q. Can male self-esteem and self-worth
be affected by the "adequacy" of a man's genitalia?
A. Yes.
- Q. Even if penile enlargement
surgery improves self-esteem, but other confidence issues
are present, should these issues be dealt with in order to change
quality of life?
A. Yes.
- Q. Are the risks of surgery low
with your technique of penile lengthening and your
technique of pericavernosal penile widening for girth
enhancement in the flaccid state and erect state with the Allograft
Dermal Matrix Graft (Alloderm ®) technique ?
A. Yes.
- Q. Can penile enlargement
surgery improve self-esteem and quality of life?
A. With respect to feelings of penile inadequacy,
meaning the man's perception that his penile size is smaller than
what he would like, even if it is normal, the results of such surgery
frequently will improve his sexual and machismo image and positively
affect his interactions with sexual partners, coworkers and professional
associates.
- Q. Because this surgery can improve
self-esteem and quality of life, and the risks with your techniques
are low is penile enlargement surgery, like other
cosmetic surgery, justified?
A. Yes.
- Q. Is penile enlargement
surgery effective with your techniques of penile
lengthening and pericavernosal penile widening for girth
enhancement in the flaccid state and erect state and glanular
enhancement with the Allograft
Dermal Matrix Graft (Alloderm ®) technique ?
A. Yes.
- Q. Is there a high patient satisfaction
rate from penile enlargement surgery with your technique
of penile lengthening and with your techniques of
pericavernosal penile widening for girth
enhancement in the flaccid state and erect state and glanular
enhancement with the Allograft
Dermal Matrix Graft (Alloderm ®) technique?
A. Yes.
- Q. How much extra length may
be obtained?
A. This is a hard question to answer, and
absolute promises cannot be made. However, length may be increased
by approximately one to two inches. Maximum length is obtained
when the surgery is combined with a penile stretching device.
- Q. How much extra width may be
obtained?
A. If you have widening by the liposuction
technique, the results are variable. I do not perform this
technique.
If you have the pericavernosal penile widening for girth
enhancement in the flaccid state by the Allograft
Dermal Matrix Graft (Alloderm ®) technique you may expect
an approximate 30% width gain (circumference) in the flaccid
state and an approximate 5 to 10% gain in girth in the erect
state.
- Q. How long
after surgery can I have sex?
A. It is advisable to wait six weeks before
engaging in penetrative sex. However, you can masturbate in approximately
1 week after surgery, as long as you use gentle masturbation of
the glans (head of the penis) only.
- Q. Are the results
of penile lengthening permanent?
A. The results of penile lengthening are
permanent, and when combined with a penile stretching device added
length will occur over time.
- Q. Are the
results of penile widening permanent?
A. The results of penile widening by
the liposuction technique are frequently
not permanent. Usually only thirty percent of the fat injected
into the penis survives after one year. In addition, asymmetry,
that is to say, unevenness and nodularity may occur. Frequently,
repeat injections (touch ups) to fill in the defects are needed.
If you have the pericavernosal penile widening for girth
enhancement in the flaccid state with the Allograft
Dermal Matrix Graft (Alloderm ®) technique the results
are much more predictable and permanent.
I have been using the Allograft
Dermal Matrix Graft (Alloderm ®) technique for 6 years
and I can tell you from my experience that the results are predictable
and appear to be permanent.
I do not perform the liposuction technique, and I
no longer perform the dermal fat graft technique.
- Q. How successful
are the treatments?
A. It depends upon how you define success.
There is a high patient satisfaction rate from penile enlargement
surgery with my techniques for penile lengthening and
pericavernosal penile widening for girth
enhancement primarily in the flaccid state with the Allograft
Dermal Matrix Graft (Alloderm ®) technique. Almost all
men who have been informed of reasonable expectations - are happy
with the results. However, it is also important that the surgical
techniques, incisions, scars and postoperative management be reviewed
and accepted preoperatively. When this is done, almost all men
are happy and consider the surgical procedures successful.
Note: There is no unevenness and no nodularity and
there are no donor site incisions and, therefore, no scars,
on the buttocks, inguinal areas, abdomen, flank or back with the Allograft
Dermal Matrix Graft (Alloderm ®) technique that I use for penile
widening (pericavernosal) and the duration of surgery
is shorter than the dermal fat graft technique. The contour is
even and cylindrical.
- Q. Is help
available without surgery?
A. There are several unproven, non-surgical
techniques of penile lengthening using penile
weights, penile stretching devices and
a vacuum pump (penis pump). The medical community does not recommend
these techniques and since no studies have been made it is impossible
to tell how effective these techniques are or what the risks might
be. Sometimes, overweight men with an increase in fat in their
pubic area will notice increased penile length after significant
weight loss. There is no non-surgical technique that produces permanent penile
widening.
- Q. What are
the risks of surgical penile lengthening
or widening with your techniques?
A. The risks of penile lengthening surgery are very low
with my technique. Very few patients experience complications.
However, I will discuss the following possible risks with you:
- a reduction of the erect upward angle
of the penis
- a keloid scar at the site of the
incision
- fibrosis of a superficial vein
- shortening of the penis
- skin incision separation
- bruising
- bleeding
- infection
- temporary swelling of the penis
- a collection of serum under the skin
- reduction in penile sensation
- erectile dysfunction
The risks of penile widening
surgery are very low with my techniques. Very
few patients experience complications. However, I will discuss
the following possible risks with you:
- absorption, contracture or failure
of the graft to take
- skin incision separation (Note: with
the Allograft Dermal Matrix Graft
(Alloderm ®) technique, there are no donor site incisions
in the buttocks, groin, abdomen, flanks or back)
- a keloid scar (Note: with the Allograft
Dermal Matrix Graft (Alloderm ®) technique, no grafts
are obtained and, therefore, there are no incisions
or scars in the buttock, groin, abdomen, flank or back regions
because there are no DONOR SITES )
- fibrosis of a superficial vein
- shortening of the penis
- infection
- bleeding
- bruising
- decrease in penile sensation
- temporary swelling of the penis
- a collection of serum under the skin
- loss of penile skin (Note: because
no donor skin is obtained with the Allograft
Dermal Matrix Graft (Alloderm ®) technique, there can
be no loss of donor site skin)
- erectile dysfunction
- Q. What can
I expect from glanular enhancement surgery?
A. Although greater than 30% increase in the
flare of the glans is possible, a reasonable expectation is between
10% and 15% increase in the flare of the proximal 1/3 of your glans
(head of penis).
- Q. How expensive
is treatment?
A. See the section on Payment
Information for complete details.
- Q. Will insurance
cover these treatments?
A. If you have a truly small penis - in medical
terms this is called micro penis - and the length measures less
than three and two thirds inches in the erect state, and the width
measures less than three and a half inches in circumference in
the erect state; or if you have had previous penile surgery, Peyronie's
disease, an aggressive circumcision, other genital trauma, or a
neurogenic bladder that caused penile shortening then insurance
will probably pay for the surgery of penile lengthening. However,
insurance will not cover the costs of these procedures when done
purely for cosmetic reasons. Insurance will not cover penile widening.
